After upgrade my development site to 1.2.9, the module positions "position-5" and "position-6" appears like the image below.

please uninstall Community Toolbar module first from Joomla extension manager > manage.
after that install latest package for Community Toolbar.

That was exactly I did and didn't work.

for position 5 and 6 is using spotlight-2, so it will appear above breadcrumb and below breadcrumb. you customize it.
here the steps:
1. goto joomla backend > extension > template manage > select socialize > layout section
2. click save as copy
