I turned on SEF Urls and URL rewriting. I also installed K2. I'm not 100% sure if it stopped working correctly after either of those things or before.

could you please elaborate more about "appropriate data"? because I cannot see any issue or failure or any broken link at notifications.

Look at the top two notifications on the image I attacked. there is a blank where there should be a name. There is also a blank where it should say "stream" or "photo".

This issue is fixed. Somehow my website was reverted back to PHP 5.3.29. Notifications and the messaging component do not work properly on PHP 5.3.29. I changed back to PHP 7.0.8 and all is well. FYI in case you see this in the future.
