In multiple profiles, if the profile needs approval, then once the administrator approves, shouldn't a notification email go out to the user. In my case this doesn't happen. Is that the way that this was intended?
The initial welcome email and the email with activation link etc works fine. Its just the notification after admin has approved, which doesn't fire.

Mail won't be sent if admin approve user in backend.
That's overall Joomla! rule.

Problem in my case seems to be that "admin" is not getting that mail. I am using acymailing to override joomla's mails and the js mails are set to "text" and not "html". Do you feel this might be the problem. I don't think anyone else has had similar problem before.

Yes, if they are overrides then yes, it may be the case.
We tested that on clean Joomla! and it works. All those emails are triggered by Joomla! and are part of joomla! registration flow.
