I was just wondering if there was a simple tick box module available for Jomsocial?

I just want it to be a one use per person up to 5 different tick boxes. Basically so it could do...

Whats your favourite colour?
Red
Blue
Green
Yellow
Pink

Then the user ticks their one. Simple!
