dating search module not designed for customization, because Jomsocial advance search still have some bug. and will fixed at version 3.3
but if you still want put extra field in dating search, you must edit the code at:
/modules/mod_datingsearch/tmpl/default.php (can using override method)
/modules/mod_datingsearch/mod_datingsearch.php (can't using override)

I already changing it to gender.

at my prev post. I attached the custom made dating search module. you can compare it with the original one.
many code changes, I can't explain it one by one.

for use that module you only need to replace the original one with that module (attached). under folder [webroot]/modules/

backup before replacing it.
