Registration Doesn't go Past Next after Upgrade! Please Fix ASAP

ISSUE SUMMARY:
I just drove a bunch of people to my site to find out that they can't go past "next" in the registration!! What a disaster! Please see my video in the Misc Info Section to replicate.

I tried updating the files in Radio-checkbox.zip file provided here w/out any luck.

Hi Jonathan,

please check it. the issue because JQuery and Bootstrap was loaded twice from community toolbar module.
I turned off it at community toolbar module. so JQuery and Bootstrap only loaded once.
