Event Cover Photo - Upload Photos Button not working

STEPS TO REPRODUCE:
Login as a event owner user or admin user
Open an event.
Click Modify Cover Photo.
Click Upload Photo tab.
Click upload Photo Button.
Local Hard Dirve window appears. Select photo to upload. Window disappears.
Nothing.

RESULT:
The upload photos button is not working when modifying event cover photo. Photo not uploading.

EXPECTED RESULT:
The photo should upload.

OCCURS ON:
Joomla 3.3.6 + JomSocial 3.2.1.6

TEMPLATE:
Socialize (Jomsocial)

OPERATING SYSTEM:
Windows 7, Windows 8.1

After upgrading to 4.0.1 users are reporting that this is not working.

Please advise ....

Nevermind. I think Browser cache was holding the default cover photo.
