Community Toolbar Login CSS On Mobile

ISSUE SUMMARY: I am using socialize template and have installed community toolbar. I noticed that when I visit the website homepage using smartphone and push the login button on the toolbar, a popup displays. But the styling is bad. Please help me resolve this issue. Thanks

STEPS TO REPLICATE:
1. Visit website home using smartphone as guest
2. Click on the login button in the toolbar in the header
3. Popup displays

RESULT Popup displayed has bad css styling(icons above textboxes and remember checkbox overlays login button)
EXPECTED RESULT Popup displays with good css styling(icons in line with textboxes and remember checkbox not overlaying login button)
BROWSER Nexus 5 - Chrome on Android
