You posted URL to login form... where's that logo mentioned?

Your supported domain is: peopla.ae
And you request support for: websitedesigninginuae.com. I'm sorry but as it is different domain, support will be not provided for it at the moment.
Unless you change supported domain (need to have valid reason e.g domain name typo)

After your request is accepted, please, update this thread and make sure it has "Pending Support" status.

You must select the proper number of questions when you create the quiz, if you edit the quiz you will see an option named Select up to: and here select the number of questions that you need to show and for this two was 1 and we change to 5

Also there was end date set for this quiz so I removed it.
